About this deal

Do not take this product for anything other than its intended use (see section 1) and never take more than the recommended amount (see section 3). Serious heart problems (symptoms of which include fast or irregular heartbeat) have been reported in patients who have taken too much loperamide, the active ingredient in IMODIUM® Instant Melts. IMODIUM® can have side effects, like all medicines, although these don’t affect everyone and most are usually mild. Get medical help at once Symptomatic treatment of acute episodes of diarrhoea associated with irritable bowel syndrome in adults aged 18 years and over feeling dizzy – if loperamide makes you feel dizzy when you stand up, try getting up very slowly or stay sitting down until you feel better. If you begin to feel dizzy, lie down so that you do not faint, then sit until you feel better. Avoid driving, cycling, or using tools or machines if you feel dizzy.

Abuse and misuse of loperamide, has been described (see section 4.9). Loperamide is an opioid with low bioavailability and limited potential to penetrate the blood brain barrier at therapeutic doses. However, addiction is observed with opioids as a class. see section 3). Treatment

Loperamide is also used for recurring or longer lasting diarrhoea from bowel conditions such as Crohn's disease, ulcerative colitis and short bowel syndrome. Your doctor will adjust your dose according to your symptoms and how well loperamide is working, up to a maximum of: Two tablets (4 mg) initially followed by 1 tablet (2 mg) after every loose stool. The usual dose is 3-4 tablets (6 mg-8 mg) daily; the maximum daily dose should not exceed 6 tablets (12 mg). The recommended dose depends on the type of diarrhoea you have and your age. Adults (over 18), with short-term diarrhoea or IBS Distribution: Studies on distribution in rats show a high affinity for the gut wall with a preference for binding to receptors of the longitudinal muscle layer. The plasma protein binding of loperamide is 95%, mainly to albumin. Non-clinical data have shown that loperamide is a P-glycoprotein substrate.

Age 12 to 17 years with IBS or long-lasting diarrhoea – follow the instructions from their doctor. This is given on prescription only. If you are constipated , or your stomach appears swollen (especially in children with severe dehydration). Occasionally patients with a colostomy (stoma) need a higher dose. Only take a higher dose if your doctor tells you to. Children's doses In acute diarrhoea, if clinical improvement is not observed within 48 hours, the administration of Imodium Instant Melts should be discontinued and patients should be advised to consult their doctor.capsules or tablets: take 2 capsules or tablets, taken immediately. Absorption: Most ingested loperamide is absorbed from the gut, but as a result of significant first pass metabolism, systemic bioavailability is only approximately 0.3%. If you have acute dysentery , the symptoms of which may include blood in your stools and a high temperature. Other ingredients are: Gelatin, mannitol, aspartame, mint flavour and sodium hydrogen carbonate. What the medicine looks like headaches – make sure you rest and drink plenty of fluids. Do not drink too much alcohol. Ask your pharmacist to recommend a painkiller. Talk to your doctor if the headaches last longer than a week or are severe. Once you're on the right dose, your doctor will usually recommend dividing your daily dose, so you take half in the morning and half in the afternoon or evening. This medicine contains less than 1 mmol sodium (23 mg) per tablet, that is to say essentially ‘sodium-free1.This medicine contains 0.00066 mg benzyl alcohol in each dose (2 mg) which is equivalent to 0.00033 mg/mg. Benzyl alcohol may cause allergic reactions. Ask your doctor or pharmacist for advice if you have liver or kidney disease, or if pregnant or breast feeding. This is because large amounts of benzyl alcohol can buildup in your body and may cause side effects (called “metabolic acidosis”). Sulphites can cause allergy like reactions, most commonly asthma symptoms in those with underlying asthma. Allergic rhinitis like reactions, urticaria and anaphylaxis might be observed. Two tablets (4 mg) initially, followed by 1 tablet (2 mg) after every loose stool, or as previously advised by your doctor. The maximum daily dose should not exceed 6 tablets (12 mg). Your body can lose large amounts of fluids and salts when you have diarrhoea. You need to replace the fluid by drinking more liquid than usual. Ask your pharmacist about rehydration therapy to replace lost salts. This is especially important for children, and frail or older people. Imodium Instant Melts contains benzyl alcohol, which may cause allergic reactions. Imodium Instants must be used with caution in patients with renal or hepatic impairment, or in patients who are pregnant or breast-feeding, because of the risk of accumulation and toxicity (metabolic acidosis).

Liquid: this comes with a measuring cup, plastic syringe or spoon to help you measure the right dose. If you do not have one, ask your pharmacist. Do not use a kitchen teaspoon as it will not give the right amount of medicine. Dosage and strength

6. How to cope with side effects of loperamide

Now read this whole leaflet carefully before you use this medicine.Keep the leaflet: you might need it again. 1 What the medicine is for Bullous eruption a (including Stevens-Johnson syndrome, Toxic epidermal necrolysis and Erythema multiforme) The tablets contain loperamide hydrochloride, a substance that helps reduce diarrhoea by slowing down an overactive bowel. This allows water and salts that are usually lost in diarrhoea to be absorbed by the body. The concomitant administration of loperamide (4 mg single dose) and itraconazole, an inhibitor of CYP3A4 and P-glycoprotein, resulted in a 3 to 4-fold increase in loperamide plasma concentrations. In the same study a CYP2C8 inhibitor, gemfibrozil, increased loperamide by approximately 2-fold. The combination of itraconazole and gemfibrozil resulted in a 4-fold increase in peak plasma levels of loperamide and a 13-fold increase in total plasma exposure. These increases were not associated with central nervous system (CNS) effects as measured by psychomotor tests (i.e. subjective drowsiness and the Digit Symbol Substitution Test).
